Across Canada, teams are working in new ways to expand access to primary care and improve how care is delivered. This investment will help them go further — strengthening the connection between quality improvement, research and evidence.
Through Care Forward, teams are already testing practical approaches, learning from one another, and making a difference in their communities. This partnership will help accelerate that work.
This is what it looks like when people, evidence and action come together to move care forward.
